Position Summary:

The UT Health Science Center-School of Public Health Brownsville Regional Campus is participating in the Texas Healthcare Transformation Medicaid 1115 Waiver program. We have implemented healthcare transformation programs that have improved access to care for the citizens of our region and are providing evidence-based healthcare. Our next phase is focused on clinic and community integration.

The Research Assistant I will work onsite with our clinic partners in Hidalgo County and will be responsible for navigating referred patients to UTHealth community services related to their chronic conditions that are designed to improve their health. She/he will conduct preliminary assessment of barriers to care, social determinants of health, and adherence to provider recommendations and medications and enroll qualified patients in services and programs.

Selected applicants will be joining a highly motivated team to assist in expanding services throughout the region using the best models of care.

Position Key Accountabilities:

  1. Work onsite in clinic to facilitate patient navigation to UTHealth community services through direct referrals (warm hand-off) and clinic referral reports as well as navigation to clinic services. Meet enrollment targets.

  2. Complete and accurate documentation of enrollment assessments, notes, and questionnaires/surveys in Wellcentive Data system and other program data tracking systems.

  3. Work closely with clinic staff to understand clinic services and workflows and facilitateclinic and community integrationefforts. Establish ongoing communication with all appropriate clinic staff and management. May participate in clinic staff meetings and/or daily huddles.

  4. Participate in data quality improvement activities, training, and service quality assurance and improvement activities. Collaborate with staff and management across sections and projects.

  5. Participate in multi-disciplinary Case Review.

  6. Provide thorough communication and status reporting to management. Comply with all governmental and University policies, rules, regulations, and codes.

  7. Performs other duties as assigned.

Certification/Skills:

General knowledge of research fields.

Preferred:

Good written and verbal communication skills in English and Spanish.

Skilled in Microsoft Officesuite of applications.

Experience in a clinical environmentand withelectronic health records.

Experience conducting surveys or questionnaires.

Familiarity with local community.

High degree of self-motivation and flexibility.

Flexibility on occasions when work schedule and location must vary.

Minimum Education:

Bachelor’s degree or relevant experience in lieu of education.

Minimum Experience:

No years of experience.

Physical Requirements:

Exerts up to 20 pounds of force occasionally and/or up to 10 pounds constantly and/or a negligible amount constantly to move objects.

This position may include work involving potentially hazardous chemical, biological or radioactive agents.
