UTHealth is looking for an experienced Certified Community HealthWorker (CHW) to join its team centrally located in the Museum District, Houston ! The CHW is a core member of the multidisciplinary care team that supports

patients and their needs post discharge in order to reduce hospital

readmission. The team is responsible for coordinating the healthcare of

patients with complex medical needs. Having a CHW in the transition of care

program will help reduce barriers to care, increase access and overall health

outcomes. This individual will be part of a centralized team providing who will be tasked with coordinating with the clinic CHW regarding new patient appointment.

Position Summary:

Work in coordination with physician, nurses and social workers to interview patients that have been enrolled or are being asked to enroll in a medical home.

Transitions of CareDuties for the central CHW Include:

  • One-on-one with CaseManager daily to discuss admissions/discharges from area hospitals.

  • Meet patients prior to discharge with new Welcome to UT toolkit (appointmentwith clinic information, transportation resources, pharmacy information),answer any questions.

  • Monitor UT patients at the TMC Emergency Center on Monday and Friday.

  • Report to Case Manage high risk findings.

  • Assist with scheduling follow-up appointments with UT scheduler

  • Reminder call to patient the day before the appointment

  • Contact no-show patients at 7/14 day post-hospitalization visit)

  • Troubleshoot barriers (housing, food, transportation, prescription assistance,insurance assistance (Medicaid etc.)

  • Access to Appointments (transportation needs met).

  • Provide access tolocal pharmacy for medication pick up.

  • Ensure DME supplies were delivered to patient.

  • Task clinic CHW regarding new patient appointment.


  • Texas Department of State Health Services (DSHS) – Certified CHW.

Minimum Education:

  • High school diploma or GED.

Minimum Experience:

  • One (1) year of relevant work experience.

  • Position Location: Binz Plaza Medical Center @ 1200 Binz Street

Physical Requirements:

Exerts up to 20 pounds of force occasionally and/or up to 10 pounds frequently and/or a negligible amount constantly to move objects.
