UTHealth - UT Physicians in the Greater Houston Area is looking for Clinical Case Managers (RN) to join their dynamic team of professionals. This position offers access to working with the UT Health / UT Physicians Clinical Practice Plan, one of the leading health plans in the world. This individual will be trained and oriented in the Texas Medical Center and function daily in one of the beautiful clinics across the UT Physicians practice .
Community oriented care providers will provide support to patients and families with complex healthcare needs, while functioning in a team environment. Clinic specialties include: behavioral health, family medicine, obstetrics & gynecology and pediatrics with a focus on community health and wellness.

Functions in a clinical role on a collaborative interdisciplinary team providing comprehensive disease management care to patients and families with complex healthcare needs. Aims to provide care that is safe, effective, patient-centered, efficient, and equitable.
Position Key Accountabilities:
Performs patient assessment and develops a plan of care with physicians and staff in a manner that promotes quality patient care and customer service/satisfaction.
Plans and implements strategies to reduce the length of stay and resource consumption, and achieve positive patient outcomes.
Manages each patient’s transition through the system and exercises judgment while coordinating care and referring patients to agencies or health care professionals for other services/transitions in care where indicated.
Acts as a patient and healthcare advocate by maintaining a safe environment and promoting quality healthcare and services.
Evaluates patients’ responses and outcomes to education and therapeutic interventions.
Communicates effectively with collaborative team while escalating critical results and issues in an appropriate and timely manner.
Acts as a clinical and leadership resource for team members. Educates the staff on case management and provides specific information related to case types.
Respects and maintains privacy and confidentiality per HIPAA guidelines.
Adheres to broad scope of practice as established by the Nurse Practice Act.
Assures compliance with quality standards, and regulatory and accrediting agency specifications to meet federal, state and local standards.
All other duties as assigned.
Minimum Education and Experience:
Four (4) years of nursing experience with a BSN from an accredited school of Nursing (preferred).
Five (5) years of nursing experience with a Nursing Diploma (three-year program).
Six (6) years of nursing experience with an ADN from an accredited school of Nursing.

Clinical Case Manager provides clinical skills of assessment, goal setting with care plans and monitoring behavior with primary focus on chronic disease and behavioral health patients.
Individual must have ability to communicate to patients through counseling and education to self manage their disease.
Must have skills in problem solving, decision making, organizational and conflict resolution. Skills in collaborating with a care team and clinical team in an patient centered medical home.
The position is primarily Monday-Friday, clinic based, collaboration with care team members, providers and clinical staff.
Paid holidays and vacation. Retirement through TRS and option to participate in 401K or 403B.
Exerts up to 50 pounds of force occasionally and/or up to 20 pounds frequently and/or up to 10 pounds constantly to move objects.
