Work in coordination with physician, nurses and social workers to interview patients that have been enrolled or are being asked to enroll in a medical home.
Position Key Accountabilities:
Establishes rapport with patients that are enrolled or that are being asked to enroll in a medical home. Motivate patients to be active, engaged participants in their health care.
Responsible for collecting data and reporting back to the care team.
Contacts patient to schedule site and/or clinical visits.
Gather information about patient’s home environment that may be affecting their medical condition. Home visits may be required.
Help patients utilize resources in their area and assist with applications for eligible programs.
Coach patients in effective management of their chronic health conditions and follow-up with healthcare plans and instructions.
Document activities, service plans, results and bill slips.
Continuously expand knowledge and understanding of community needs, resources, services and programs available.
Maintain confidentiality per HIPAA guidelines in regards to patient information.
Performs other duties as assigned.
Texas Department of State Health Services (DSHS) Certified CHW.
Valid driver’s license required.
Exerts up to 20 pounds of force occasionally and/or up to 10 pounds frequently and/or a negligible amount constantly to move objects.
