UT Physicians Family Practice Clinic - Texas Medical Center is hiring a Bilingual (English/Spanish) LMSW to join their team. The Clinic provides care to a patient population with a diverse lifespan (pediatrics up to geriatrics). Ideal candidate would have experience working with all ages.
Themedical social workers collaborate with the interdisciplinary team within theclinic to address the patient’s psychosocial needs. Medical social workers workclosely with patients and their family members who are experiencing mental,emotional, family, and/or financial stressors.
of the services routinely offered by social workers in the Family Medicine Clinic include the following:
· Perform a comprehensive biopsychosocial
assessment of patient.
· Assess for abuse, neglect or
exploitation of protected classes.
· Conduct home visit toensure that a
patient’s home environment reinforces their healthcare needs.
· Assist patients with obtaining higher
level community resources.
· Informs other members of the health care team
about biopsychosocial factors which may affect the patient’s health and well-being.
· Work with the patient, their family/
support systems and provider to develop a plan of care for the patient in his
or her home or environment.
· Explain health care resources and
polices to patients, family, and professional staff.
· Lead support group discussions,
provide individual counseling.
· Provide support to patients with
serious/chronic illnesses or new diagnosis.
· Provide psychoeducation.
· Assist patient and families by
ensuring that they receive the appropriate vetted resources and referrals
needed to meet their identified needs.
· Promote patient’s self-advocacy skills
to enhance the outcomes of the patient’s care and quality of their health.
· Advocate for the needs and the
interest of the patient and their support system in order to improve their
health outcomes and access to care.
· Social workers may help patient with
advance directives and other long term care issues.
· Assure that patient’s emotional and
social needs are met.
· Provide crisis intervention: assess
the situation, deescalate, and refer asappropriate.
Provides professional social work services on a collaborative interdisciplinary team providing comprehensive medical and mental health services and support to interpret social and environmental factors relating to treatment and/or research.
Position Key Accountabilities:
Interviews and screens patients to interpret social and environmental factors and medical requirements.
Plans course of treatment or refers cases to appropriate professionals, institutions or agencies to ensure that patients receive appropriate treatment or referral.
Obtains data from patients and/or relatives in the best interest of their problems to assist in the development of a number of significant teaching endeavors, programs, and social services.
Works towards meeting total patient care and handles all aspects of routine casework. This includes interviews with the patients and/or relatives to study the individual’s problems and needs, confers with the physician, faculty, and/or with any social agency which may have an interest in the patient’s problem, assists patients in developing an economically feasible method of planning for services, and to make psychological and social adjustments necessitated by different types of illnesses, and conducts individual and group therapies.
Prepares detailed reports and may video-tape sessions with patients for review with coworkers or a faculty member.
Solicits the assistance of religious, social, and welfare agencies in the treatment of patients. Interacts with social work staff and psychiatric residents.
Responds to emergency situations.
Performs other duties as assigned.
One (1) year of social work experience required ( post-licensure ).
Medical social work experience is preferred.
Bilingual / Spanish preferred for clinic to meet the needs of the patient population.

Opportunity to work with a Provider dedicated to patient care and integrated wrap around services for the patients.
Exerts up to 50 pounds of force occasionally and/or up to 20 pounds frequently and/or up to 10 pounds constantly to move objects.
